The Vial of Sacred Water is a quest item exclusive to Qimah Reservoir in the Interlude 2 chapter: the one you reach after finishing Act 4 of the Recruit the Maraketh quest. It is technically not tied to any main quest objective. The game does not tell you to find it. The game does not explain what it does. Classic path of exile 2 secrets.
There are exactly two vials in the entire area. Each one is worth one rare currency item: usually an Exalted Orb. Together, they are the easiest two Exalted Orbs you will ever earn, assuming you do not die to a slug.
Two sources. Both in Qimah Reservoir. Neither is marked on the map.
Drops from any monster in the reservoir. Drop rate is unknown, which is GGG's charming way of saying "kill everything." The poe2 Qimah Reservoir vial drop is random but consistent enough: clear all mob packs you encounter before proceeding and you will likely see it.
Guaranteed. Find a room containing a large urn. Interact with the urn to trigger Bloodbilge, a named mini-boss. Kill it. Open the chest behind it. The vial drops alongside other loot. There is a waypoint checkpoint just outside this room: a courtesy, given that the game apparently anticipated your failure.
Warning If you die to Bloodbilge after looting Vial #1 and quit before grabbing the chest: the chest may not respawn. Known bug. Move accordingly.
Locate the Sacred Wells scattered throughout Qimah Reservoir. They look like hexagonal stone fountains with a pipe in the center. Due to procedural generation, their exact positions shift between runs: the game respects your time about as much as you'd expect.
Rule One vial per well. Two vials, two wells. You cannot double-dip the same well. Find both wells. Use both vials. Do not leave one behind like a tourist.
The sacred well poe2 reward is not always an Exalted Orb: the wiki confirms it can also drop a Vaal Orb or other rare currency. Exalted Orbs appear to be the most common outcome. Either way, the vial of sacred water reward is worth the thirty seconds it takes to interact with a fountain.

No. Two is the maximum. One random drop, one guaranteed chest. That is all GGG budgeted.
