The engine is simple and sadistically elegant. Hematolagnia reclassifies Blood Wave as a Core skill, stripping its cooldown. Your entire job is maintaining Essence to keep casting it. Skeletal Warrior Defenders generate Blood Orbs automatically, which get consumed without your involvement. Corpse loops run passively via runewords. You are a spectator to your own genocide.
Overpower stacking: amplified by Red Blessing and the Fer of the Blood Binder Charm Set: is the damage vector. Each cast of Blood Wave can deal hundreds of billions per hit, pushing into trillion-DPS territory with proper gear. Iron Maiden restores Essence on hit. Decrepify is applied automatically by the Wat runeword. You are not building a character. You are constructing a machine.
The Necromancer does not fight. The Necromancer presses a button and watches physics happen to other people.

This guide assumes Level 70 and Torment 1 unlocked. Getting there: level with Blood Surge + Blood Wave combined until Kessime's Legacy is acquired. Run Skeletal Mages with Gift of Death for Essence sustain in the interim. Once you have Kessime's Legacy with Essence per Second on Boots and Chest, transition fully to Blood Wave spam.
The Necromancer leveling path through Hematolagnia requires Core Skill Ranks investment early. Prioritize them on every piece of gear. Without enough Core Skill Ranks, Blood Wave damage scales poorly until mid-Torment. Do not skip this. Everything else is Diablo 4 character progression.
