A giant land-shark lives on an island in Act 4. You kill it. It drops a fin. A village healer wants that fin. You give it to her. She hands you a gem. Done. This guide exists because the game refuses to explain any of this to you.
The Tribal Medicine side quest is triggered the moment you pick up the Shark Fin quest item. The item's tooltip reads: "The sea gave it strength. Now it serves a greater purpose." The game then tells you to "find a use for it" and offers zero further direction. Welcome to Path of Exile 2.
Act 4 is an archipelago. You need a ship. Talk to Mikoru in Kingsmarch, hire the boat, and find Whakapanu Island on your map: it's in the northern cluster. The island is roughly level 51 content. If enemies are erasing you, that's a you problem.
Runes of Aldur note: In patch 0.5, Ezomyte Remnants appear across Act 4 islands. Activate them for rune recipes or raw currency: empowered monster waves spawn either way. Don't ignore them while hunting the Shark Pit.
Whakapanu Island maps are randomized every instance. There's no fixed coordinate for the Shark Pit: it's a large, circular sandy arena open to the water. Walk the map perimeter until you find it. The arena will be obvious: round, sandy, ocean-adjacent, ominously empty.
The Great White One does not spawn on zone entry. This is where most players stand confused in an empty arena and type in global chat. Walk to the water's edge inside the pit. The boss will emerge from the sand. Yes, it swims through sand. Yes, that's stupid.
Bug: Some players report the boss not spawning at all on first entry. Exit the zone and re-enter. The arena resets. This is a known issue going back to patch 0.3 and GGG has not fixed it with conviction.

The boss deals physical damage only: your elemental resistances are irrelevant here. Movement speed is your friend. Ranged builds trivialize this. Melee builds will eat sand repeatedly and deserve it.
After looting the Shark Fin, your inventory now has a 2x2 item occupying premium real estate. The quest objective is "Find a use for the Shark Fin." The NPC you want is Kaimana, a tribal healer. Her location shifts based on your Act 4 story progress. This is by design. It is also annoying.

Ngakanu Island is the first island directly south of Kingsmarch on your map. Sail there, walk up the beach east, and she'll be in the settlement. The interaction with Kaimana on Ngakanu is the fastest route: she recognizes the fin and the dialogue resolves instantly.
Kaimana invisible on Kingsmarch: The Karui ship is not always docked. If you go to Kingsmarch and the ship is absent, Kaimana has temporarily relocated to Ngakanu. Complete more of Act 4's main quest to bring her back.
Great White One arena empty: Boss fails to spawn in some instances. Leave zone, re-enter. If persisting, relog. Multi-instance fresh spawns solve it in all documented cases.
Quest item stuck in inventory: Players who rush through Act 4 before visiting Ngakanu sometimes find Kaimana unreachable until main quest completion. Finish The Search quest: she will appear on the Karui Ship in Kingsmarch afterward.
The Shark Fin quest itself is unchanged in patch 0.5. What is different: Act 4 islands now contain Ezomyte Remnants as part of the Runes of Aldur league mechanic. These interact with Runesmithing recipes: combining specific rune types to craft currency and upgrades. The combination of a Tempest Rune and Lightning Rune produces an Orb of Augmentation, for reference.
While farming Whakapanu Island for the Great White One boss fight, you'll encounter Remnants offering rune selections. Take them. The empowered wave of enemies that spawns afterward is manageable at level 51, and the currency rewards offset the time spent running the island. Over 150 rune types exist in 0.5: treating Act 4 as pure transit is a waste.
Efficient path: Land on Whakapanu: clear Remnants along the route: find Shark Pit: kill boss: portal back: sail to Ngakanu: hand fin to Kaimana: return to Whakapanu for any remaining content. Minimize ship trips.
