Brutality is the seasonal reputation currency of Diablo IV Season 12: Season of Slaughter. Congratulations: you now grind a resource called "Brutality," in a season themed around a demonic serial killer, to unlock a board that rewards you for mass murder. Blizzard is doing great.
You earn Brutality by completing Killstreaks: chains of rapid kills tracked by the game's new momentum system. The more enemies you slay without pause, the higher your Killstreak Tier, and the more Brutality lands in your pocket at the streak's end. Elites and Champions also drop Brutality directly when slain during active streaks.
All of this feeds into the Seasonal Reputation Board, which has 24 tiers. Complete all 24 and Blizzard rewards you with a Resplendent Spark: the crafting material for Mythic Unique items, which, yes, is exactly as rare and agonising to use as you'd expect.
After completing all 24 tiers, the board doesn't stop: it just starts coughing up Greater Bloodied Caches every 12,500 Brutality. You're never actually finished. Welcome to ARPGs.
These Greater Bloodied Caches contain: gold, Fresh Meat, Pounds of Flesh, Bloodied Lair Boss Keys, Bloodied Nightmare Dungeon Sigils, and Ancestral Bloodied gear. In short: the cache is the loop that feeds the loop that feeds the loop. Infinite hunger. Like The Butcher himself. It's almost poetic if you ignore the smell.
Before any of this matters, you must complete the seasonal questline A Taste of Power, which begins in Gea Kul on the Seasonal Realm. The quest auto-starts the moment you log in with a seasonal character. Follow the markers, speak to NPCs, and get handed a cleaver by the plot. Takes roughly 20 minutes. You'll fight a Butcher at the end, which is a perfectly reasonable thing for a Tuesday.
Players using Vessel of Hatred must complete the expansion's prologue first. Everyone else: Gea Kul, Season tab, go. If you skip even this, nothing else in the guide is accessible.
Completing the questline unlocks the full Brutality Reputation Board, access to Slaughterhouses in Gea Kul, and the ability to interact with Shrines of Slaughter in Helltides. It rewards a Bloodied Cache containing a handful of starter keys and sigils. You will blow through these in approximately four minutes and then need to farm more. This is your life now.
Brutality is the seasonal reputation currency of Season 12. You earn it by chaining kills into Killstreaks and by slaying Elites and Champions that drop it directly on death. It fills your Seasonal Reputation Board: 24 tiers total.
Complete all 24 tiers and receive a Resplendent Spark: the crafting material for Mythic Unique items. After that, every 12,500 Brutality awards a Greater Bloodied Cache containing Fresh Meat, Pounds of Flesh, Bloodied Sigils, Bloodied Boss Keys, Ancestral Bloodied gear, and gold. You never stop needing Brutality. This is intentional.

Reason 1: Killstreak chaining is trivial here. The Pit's dense, room-to-room layout lets you maintain long unbroken Killstreaks with zero dead time between packs. The higher your Killstreak tier when it ends, the more Brutality you bank. Reaching Blood Bath: the max tier: is routine here with any competent speed build.
Reason 2: Champions and Elites drop Brutality directly. The Pit spawns heavy numbers of Blue-aura Champion packs (groups of 3–5) and Yellow-aura Elite enemies (single targets). Both types award Brutality per kill on top of your Killstreak multiplier: it's double-dipping on every run.
Your primary target is always the Champion pack. A pack of five at Torment 4 is worth 300 Brutality total: equivalent to two Elites. Don't skip them. Don't backtrack for an Elite if it costs you the Killstreak.

At Torment 4: three complete Pit runs generate approximately 12,500 Brutality: one full Greater Bloodied Cache. A speed build clears Pit T55 in under two minutes. That's roughly one Cache per six minutes of play.

Pit T55 gives the exact same Brutality as Pit T99. Running higher tiers for Brutality is pure time waste. The only reason to push above T55 is Paragon Glyph upgrades: and only that.
Players farming Brutality at T80 are earning identical rewards at 3–4× the time cost per run. Lock in T55, clear it in under two minutes, collect your Brutality, and repeat. This is the entire strategy. Everything else is decoration.

Pit T25 (Tournament 2) is perfectly viable. The Brutality values are roughly 60% of T4 values per enemy, meaning you'll need about 5–6 runs instead of 3: but T25 runs are also shorter since enemies have significantly less health. Net efficiency is lower but not dramatically so.
The rule is always the same: farm the tier your build clears fastest, not the hardest. A clean 90-second T25 run beats a laboured 4-minute T55 run on Brutality-per-hour every time.
