The Echo of Mephisto, also called the Echo of the False Prophet, is locked behind Torment X. Not Torment IX. Not "almost there." Torment X, or the Lord of Hatred keeps his doors shut and his loot firmly to himself. Found at Hanged Man's Hall in eastern Kehjistan near the Tarsarak waypoint, this is the fight you have been grinding toward: or dreading.
Unlike the campaign encounter, Uber Mephisto arrives with three phases, each containing at least one attack that is a guaranteed one-shot regardless of your armor, resistances, or delusional confidence in your build. Every phase escalates. You will not outstat this fight. You will either move, or you will die.
Crux note: The Crux of the False Prophet is consumed after the kill to unlock Mephisto's loot hoard. You do not need it to enter or practice the fight. Dying does not cost you the key. Opening the chest without killing him first does, because that is not how it works.

On DPS: If you are not killing fast enough, the Echo of Mephisto phases will cycle through their hardest attacks more frequently. The fight punishes slow players twice: once with more attacks, once with the shame.

Before Mephisto drops the pretense entirely, you fight his human disguise: Akarat. The Echo of Mephisto phases begin here, and this one is the warmup. He telegraphs most attacks from his front in straight lines. Stay behind him. That is the entire Phase 1 strategy condensed to four words. The rest is just survival tax.
Phase 2 opens with an immediate barrage of fireballs. There is no loading screen warning. Run. Basic attacks become light beams and red lightning: both easily dodged by staying behind him, a lesson Phase 1 taught you whether you paid attention or not.
This is where the Diablo 4 Lord of Hatred boss strategy gets genuinely punishing. The poison waves require precise timing and the bullet-hell finale is designed to overwhelm players who think "fast build" means "reckless."

The moment you enter the Phase 3 arena, Mephisto launches an automatic OHKO: a circular moving wave. It does not care about your HP. It does not care about your DR. Hug the south edge immediately. This is the Echo of Mephisto OHKO mechanic in its purest form: positioning or death, no other choices.
After surviving the welcome gift, white orbs spawn around Mephisto. Collect them by circling him. They are not optional. The arena will be littered with slow-moving projectiles for the rest of the fight. Stay behind Mephisto at all times. He slams and swipes in his facing direction, and Phase 3 is where impatience gets deleted.
The Mephisto loot hoard Diablo 4 reward pool is the reason you are here. His chest at Torment X and above contains Mythic Unique Seals, Mythic Unique items, and crafting materials that justify the entire key economy. The game specifically flags him on the map as having elevated drop probability for Mythic Seals: one of the few honest things it tells you.
Echo of Mephisto Torment farming is not a daily activity. Cruxes are throttled by design. Treat each run as a significant investment: do not attempt him at a tier where you are barely surviving. Lower-Torment kills produce lower-tier loot, and a wasted Crux is a wasted run.
