Congratulations, you installed the patch. Now sit down while I explain why your first three hours were wasted wandering the overworld like a lost goat. Diablo 4 Season 14 revolves entirely around one mechanic, and if you skip it, you're playing the season wrong on purpose.
Pandemonium Ruptures are rifts that tear open between Sanctuary and Pandemonium, dumping a fresh monster family called the Risen on your head along with floating tears you're supposed to close. Kill the cultists guarding a Death's Head Idol, the ritual circle pops, enemies flood in. The longer the rupture survives, the fatter the reward. Simple. Even you can manage it.

The overworld farm known as Helltide is still doing the heavy lifting here, since surging ruptures replace its old world events and are noticeably more aggressive about it: the tears actually chase you now instead of sitting politely on the ground.
A Realmwalker is the boss you get for actually finishing a rupture instead of half-assing it. Watch for the blue orbs spinning around it, dodge the delayed-explosion pools, and destroy the Empowering Spires at 66% and 33% HP or it simply refuses to take further damage, because bosses in this game love a good invulnerability phase.
Kill it, get a portal. That portal is the entire point of this exercise.
The portal drops you into a Deathtoll Chamber, a one-room arena with a pillar in the middle. Activate it, survive 60 seconds, expand the ring to the wall, collect your reward. Not complicated, unless you ignore the Gravehound orb mechanic, in which case the Exarch eats it and turns into a bigger problem than the timer.

Whatever you're doing in a Nightmare Dungeon this season, pray it rolls the Rupture affix, because that turns a normal dungeon clear into a two-for-one deal without you lifting an extra finger.
Chambers drop Superior Lair Keys, the new top-tier key, plus Betrayer's Husks. Both feed into unlocking the season's Lair Boss cache, so hoarding keys and doing nothing with them is the endgame equivalent of leaving loot on the ground out of spite.
The Corrupted Reaper sits at the Pandemonium Threshold near Zarbinzet. You have to run the season questline to Torment 1 first to unlock it: yes, the one system this season that actually gates behind story content, so stop saving your quest log for "later" and just do it as you go.
His Hoard has the best direct odds of dropping a Mythic Unique in the game right now, plus Pandemonium Fragments, the crafting material you feed into the Horadric Cube alongside an 850+ item power Unique to force a Mythic roll manually.
Quick loop, memorize it: find rupture → keep it open → kill Realmwalker → clear Chamber → stack keys → punch Reaper → repeat until your inventory begs for mercy.
Until level 25, War Plans only offer Helltide or Nightmare Dungeon nodes. Take Helltide every single time. No exceptions, no "just this once." Rupture density inside Helltide is the fastest experience source at any level right now, full stop.
Season reputation caches scale to your character level. Open them early at your own risk and you'll be handed garbage. Sit on them until you hit max level, then crack them all at once for an actual gear spike instead of a participation trophy.

Arcane rifts spawning across Sanctuary that summon Risen monsters and tears; killing enemies and closing tears keeps rewards climbing.
Complete a Surging Rupture with Mastery for a chance, or finish a Colossal Rupture at Zarbinzet for guaranteed spawn.
Superior Lair Keys, the main currency for opening the Corrupted Reaper's Hoard, plus Betrayer's Husks and bonus loot drops.
Complete the Season 14 questline through Torment 1; after that it becomes a repeatable Lair Boss near Zarbinzet.
Take rigid Helltide War Plan nodes until level 25, then stack Maggots and Meat Brothers nodes for exponential experience gains.
